Semaglutide vs. Tirzepatide: Which Weight Loss Medication Is Better?
The two most effective classes of weight loss medication available today are semaglutide (Wegovy, Ozempic) and tirzepatide (Zepbound, Mounjaro). Both are once-weekly injectables. Both work on GLP-1 receptors. But they differ in mechanism, efficacy, and side effect profile in important ways.
Mechanism of action
Semaglutide is a GLP-1 receptor agonist — it activates one receptor pathway (GLP-1) to reduce appetite, slow gastric emptying, and improve insulin sensitivity.
Tirzepatide is a dual GLP-1/GIP receptor agonist — it activates two receptor pathways simultaneously. The addition of GIP (glucose-dependent insulinotropic polypeptide) activation appears to enhance fat metabolism, improve muscle preservation, and reduce gastrointestinal side effects compared to GLP-1-only agents.

Why does the dual mechanism matter?
GIP receptor activation does several things that GLP-1 alone doesn't:
- Enhanced fat oxidation: GIP signaling in adipose tissue increases the rate at which your body burns stored fat for energy
- Muscle preservation: The GIP pathway helps maintain lean muscle mass during weight loss — critical for long-term metabolic health and avoiding the "skinny fat" outcome
- Better GI tolerability: GIP appears to buffer some of the gastrointestinal effects of GLP-1 activation, explaining the dramatically lower nausea rates
- Improved insulin sensitivity: Dual activation provides more robust improvements in blood sugar regulation
Which should you choose?
For most patients, the clinical data favors dual-agonist formulations: better weight loss, fewer side effects, and better muscle preservation.
